Error when trying to create a new forum

I uploaded / installed / configed everything correctly but I get this error when trying to create a new forum:

Now I'm getting this error:

I reinstalled the script, but image verification is broken and I can't edit the settings or I'll just end up with the same problem as above.

I don't think it's incorrect. I am still looking into the cause of this error, it's really weird that it only happens to some people.
